Documents Needed

To ensure a smooth application process, please gather the below documents and items before visiting the Passport Services Office at Texas State University. 

 

Each application will need the following items listed below

 

1. Application

  • Completed the application in advance of your appointment
  • Completed in black ink or via Form Filler
  • Printed single-sided

2. Proof of U.S. Citizenship

  • Original and one B&W photocopy submitted with application
  • Original will be returned to applicant via USPS by U.S. Department of State. Separate from the new passport.

3. Proof of Identification

  • Valid State of Texas ID
  • Color copy of front and back of ID (single-sided)

4. Photo

  • A 2x2 photo with white background, no hats, glasses, etc. More information can be found at here.

Note: You may choose to take your passport photo on site at Texas State University, if you do not already have one. This service is provided in the Passport Services Office.

5. Payment

  • NOTICE: Kindly be aware that the Execution Fee/Acceptance Fee will be paid to Texas State University with card only during the appointment.
