Clearly state the purpose and scientific merit of the proposed research. Provide evidence that this research has not been performed before, or if it has, then explain why it must be duplicated. Please include appropriate citations *

Describe the methods and materials that will be used in your research. Provide specific protocol that address how the samples will be collected. Clearly describe the specific tissues and amounts to be collected. For instance, specify the specific bone, part of the bone, and weight of bone to be removed. *

Provide evidence of your experience and competence in the proposed protocol. Also discuss how the research will be funded and the availability of equipment necessary to conduct the analyses. *

Justify why the tissues requested are critical for the proposed research. Explain why other elements, non-osseous, or non-human materials cannot be used. Explain why FACTS collections must be used. Are other collections also being used? If so, which collections? *

Provide documentation that the sampling procedures are the most non-destructive available to accomplish the study. Also indicate if unused samples will be returned and how they will be damaged. *

Explain when and how the data collected from the destructive analyses will be provided to FACTS. Generally data must be provided within one year of collection whether the research is complete or not. Once provided to FACTS, the data can be distributed at the discretion of the FACTS Director *

If your institute requires IRB approval, discuss if approval is granted and provide appropriate documentation. Discuss how you will protect the identity of donors and donor families. *
