Capstone: Substitution Form
If your major already has a capstone (or capstone-like) graduation requirement, you may be able to get credit also for your Honors Capstone. Use this document to submit your appeal to the Honors College. Form downloads best using Firefox or Safari.

Classroom Visit Request
Instructors may submit this form if they would like an Honors College representative to visit their class to share a glimpse into the Honors expectations, opportunities, and benefits of membership.
Instructors may also use this same form to request a visit for election and voter registration information.

Contract Course: Assessment Form
The Honors Contract Course allows an honors student to enroll in a 3000 or 4000-level non-honors course, complete additional work that represents a qualitative enhancement to the existing course, and have the completed course with contracted work count for honors credit. Faculty should use this form at the end of the semester to report on the student's contracted Honors work.

Contract Course: Proposal Form
Students seeking Honors credit for a non-Honors course should submit this form by the 12th class day of the long semester or the 4th class day of a summer session. Please discuss your proposal with the relevant faculty member prior to submitting this form.

Cross-Cultural Experience (CCE): Pre-Approval
Students with a Minor in Honors Studies must complete a Cross-Cultural Experience (CCE). More information about the various pathways to completion can be found on the CCE website. The approval form must be submitted prior to beginning work on the CCE; proposals will not be approved retroactively.

Cross-Cultural Experience (CCE): Reflection
Students with a Minor in Honors Studies who have completed work on an approved Cross-Cultural Experience (CCE) must submit a brief reflection that explains how their CCE met or exceeded our expectations. This form should only be submitted after the CCE has been completed, and it must be submitted prior to final graduation processing.

Departmental Approval Form
Faculty members wishing to teach an Honors College course must submit approval from their home academic unit using this form. Ordinarily, approvals are initiated by the Honors College; but faculty may use this form in cases where details need to be changed or revised, or when proactive approval is sought. Dismissal & Probation: Honors College Appeal
Students who would like to appeal their dismissal from the Honors College must submit this form. Your response will allow us to consider mitigating circumstances and potentially allow you to continue your affiliation with the college on probation.

Faculty & Staff Award Nominations
Student nominators should use this form to nominate outstanding faculty & staff in three different award categories. Results are announced at the end of each school year.

Honors College Travel Stipend Request Form
The Honors College encourages students to travel and present at conferences. Active Honors College students may request travel funds!

Minor in Honors Studies: Add or Remove
Use this form to add or remove the Minor in Honors studies from your degree plan.

New Honors Course: Curriculum Substitution Form
Faculty proposing a new course in the Honors College must seek prior approval in order for the new course to substitute as or count toward another (non-Honors) area of the curriculum. This form must be completed by the faculty member and the department chair / program director for the area of substitution. Completed forms should be sent to the Honors College.

New Honors Course: Proposal Form
Faculty submitting a new course proposal must use this form to provide background information and details about their proposal. Particular attention should be paid to course outline (which should include detailed topics and activities for each week) and the bibliography (which must be in an appropriate citation guidelines). The short description on this page will become publicly available in the course catalog. Completed forms should be sent to the Honors College.

Every semester the URF committee awards between 5 and 10 competitive scholarships of up to $1000 to students seeking funding for their projects.
